FAMILY LEAVE & MEDICAL LEAVE
Under the Family and MedicalLeave, regular employees are eligible to request up to 12 weeks (84 calendar days) of leave, including unpaid leave, when any of the following events occur:
- birth or adoption of the employee's child, or placement of a foster child with the employee.
- serious health condition of a child, spouse, or immediate family member, or self. 'Immediate family member,' for the purpose of this policy, is defined as: parent, parent-in-law, sister, brother, child or individual permanently residing in the same household.
Leave may be taken all at once or it can be taken in parts. An extension may be granted for up to an additional
12 weeks (84 calendar days) with proper documentation and the approval of the supervisor and department director. Documentation Required: certification from a physician or physician's assistant. The certification shall include diagnosis and duration.
