X-treme Level III (12^th grade - 3^rd grade)
Location: Scott Rakow Youth Center, 2700 Sheridan Avenue
Mondays & Wednesdays; 6:00 pm - 8:00 pm
Annual fee: $300 res. ($450 non-res.)
*X-treme program consists of cheer, tumbling practices, community service, exercise/fitness and more. Additional fees and practices may be required for uniforms and competitions. Practice times may change. Season runs from September 2012 - April 2013.

X-treme Level II (12^th grade - 3^rd grade)
Location: Scott Rakow Youth Center, 2700 Sheridan Avenue
Mondays & Wednesdays; 4:30 pm - 6:00 pm
Annual fee: $275 res. ($415 non-res.)

Proof of Residency Requirements
One of the following: Lease or property tax statement AND Two of the following: Picture ID, voter’s card, drivers license, utility bill. All proof of residency must match. Plus a copy of participant’s report card for all program locations. We may require additional documentation.

Enrollment Cards
Participants presenting enrollment cards MUST show proof of residency and may be required to fill out additional forms.
